Speaker: Mark Naghsh, founder Affinial Care
Mark is the founder of Affinial Care and is a Masters level Social worker who is also a Certified Geriatric Care manager with over 25 years experience in health care. Specializing in complex chronic care, he is a passionate expert in all phases of the human management life-cycle. From engagement to client-centered approaches, his career has always been about the betterment of a system via a strategic view of how to use core strengths of a client or family system to improve upon the care for those he serves.
Objective: Case studies and Literature review using examples of the preventative model and its impact on reduction in hospitalizations and thereby positive outcomes
