Dana R. Deravin Carr, RN-BC, CCM, Dr PH©, MPH, MS, is a NYS licensed Registered Nurse and is certified in case management through CMCC and ANCC since 2002. Dana provides professional care management to acute care patients on the Rehabilitation Unit, the Burn Unit and the Surgical Intensive Care Unit at Jacobi Medical Center, a leading Level 1 Trauma Center in the Bronx, New York. As a member of the Care Management team at Jacobi, Dana demonstrates consistent growth and leadership as she applies care management principles and standards to her daily practice. Dana has worked with her social work colleagues to facilitate the successful implementation of the Care Manager/ Social Work Dyad Team in Critical Care and In-Patient Acute Rehabilitation.
Since 1975, Ms. Deravin Carr has held a variety of clinical and administrative positions in acute care, critical care, home health care and long term care. She has published in the journals of "Professional Case Management" and "Nursing 2008" and published a book chapter for the Handbook of Research on Information Technology and Clinical Data Administration.
In addition to her work at Jacobi Medical Center, Dana has lectured at Mercy College and presented programs on case management for the SUNY New Paltz Nursing Program, the RC Picker Patient Centered Care Institute and the Hudson Valley Chapter of CMSA. Ms. Deravin Carr is an editorial board member of "Case and Point:" Magazine and has served on various tasks forces for CMCC and ANCC.
Currently, Ms. Deravin Carr is a doctoral candidate at the New York Medical College * School of Public Health. She holds a Masters Degree in Public Health and a Masters in Science * Gerontology, and was a 2008 Fellow at the NYU College of Nursing Leadership Institute for Black Nurses.
Veronica Chepak - Montefiore CMO's Directors' Award for the fouth quarter of 2010
Our Hudson Valley CMSA President Veronica Chepak was given the Montefiore CMO's Directors' Award for the fouth quarter of 2010. "Veronica was nominated for the outstanding work she does in support of our members and patients each and every day! Kris Becker wrote about Veronica telling us, "Veronica is an exemplary employee at the CMO. She leads by example and always makes her co-workers strive for excellence." She is the type of individual who goes above and beyond every day! She teaches her co-workers, shares her knowledge and experience and works tirelessly for her patients.
In addition to the incredible work Veronica performs each day at CMO, she is also the President of the Hudson Valley CMSA branch. Veronica ensures that CCM training is available to any Montefiore CMO Employee. Through her leadership position, in the CMSA, she supports the CMO case managers and all case managers in this region. Veronica uses encouraging words and a positive attitude to encourage others to strive for excellence and overcome barriers and obstacles. Veronica sets the example and she helps to provide an positive environment for all.
Susan Herzog - Public Policy Liaison

Susan Herzog, B.S.R.N is currently the Project Supervisor of the Westchester Cares Action Program at Hudson Health Plan. She oversees the day to day operation of all outreach and case management for this Chronic Illness Demonstration Project (CIDP). She has over 12 years of Case Management experience and spent 10 years as an Executive Sales Manager in Managed Care for Workers Compensation. Prior to entering the into the Managed Care arena, Susan practiced as a Registered Nurse in the community and in various hospital acute care settings.
Public policy Case managers are integral members of the care coordination team and viewed as advocates for consumers. As such, they need information to be better able to educate consumers and respond to issues that affect consumer's access to quality health care services.
CMSA's Public Policy Committee maintains information and resources that Policy Makers and Advocates can utilize to keep up-to-date, informed, and educated regarding current issues and trends happening in the world of health care.

Congratulations to Teri Treiger who is now the National CMSA President 2010-2011

Teri M. Treiger, RN-C, MA, CCM, CCP, has 20+ years of care management experience; and has worked in managed care, acute hospital, and rehabilitation hospital settings, as well as served as a clinical consultant for a large medical management company advising on coding and reimbursement issues. Teri's current focus is the design of transition of care and complex case management programs. In 2008, Teri worked with a healthcare system in Alabama to establish an uncompensated care program, which leveraged medical management in collaboration with practice-based case management to educate patients about their healthcare condition, coordinate necessary care, and identify alternative funding sources. Teri has devoted many years as a member of the CMSA Board of Directors; and is the current President (2010-2011).
Hudson Valley Chapter Case Manager of the Year 2010 - Diane Lantz

Diane Lantz, RN, is Assistant Director of Clinical Care Management at Montefiore Medical Center. Supervising Care Management Coordinator
RN’s, Diane’s position spanned two campuses and involved managing 20 RNs. Before joining Montefiore, Diane was involved with case management activities at Lawrence Hospital Center, Phelps Memorial Hospital Center, Sound Shore Medical Center, Bell Atlantic and at Managed Comp for it’s workman’s compensation benefits. Her extensive experience in the hospital setting laid the foundation for her success in her present role at Montefiore, a 1500 bed academic medical center. Her staff respects her expertise as she assists both them and the Social Workers in their discharge planning activities for very complex patients. Diane is a teacher for her staff and the entire care management team. She always goes above and beyond in her efforts to ensure that patient needs are met both in the hospital and once they transition after discharge.

Catherine M. Mullahy, RN, BS, CRRN, CCM, is a nationally recognized Case Management Consultant and Speaker. She is Founder and President of Options Unlimited, a medical case management and benefits consulting firm in Huntington, New York, serving corporations, insurers, unions, townships, and third party administrators nationwide since 1983. With over 36 years of experience managing health care delivery in home, hospital, hospice, and critical care settings, her diversified clinical background includes treatment of multiple trauma, cardiac, orthopedic and neurological conditions. Ms. Mullahy is a past president of the Case Management Society of America, and was named 1999 Distinguished Case Manager of the Year by the Case Management Society of America. She is past Chair of the Commission for Case Manager Certification (CCMC), sits on the CCMC Executive Committee, and has served on ongoing expert panels in connection with the development of the CCM credential since its inception. Her book, The Case Manager's Handbook, Second Edition was published by Aspen Publishers, Inc. in October 1998. It was awarded as a 1998 "Book of the Year" by the American Journal of Nursing Magazine. Editor of The Case Manager and contributing editor for Case Management Advisor, Ms. Mullahy is on the editorial board of Home Health Care Dealer. She is also contributing editor for Medical Case Management - Forms, Checklists & Guidelines (Aspen Publishers, 1995), and a contributing writer to Clinical Care Management in Managed Care (Aspen, 1998), The Managed Health Care Handbook, Third Edition (Aspen Publishers, 1996) and The Persons Served: Ethical Perspectives on CARF's Accreditation Standards (CARF, 1997).
